Referral Information
What Happens After You Refer?
- Referral Review - Our team reviews the information provided and confirms receipt of the referral.
- Initial Discussion - We contact the participant or relevant representative to discuss their circumstances, goals and support requirements.
- Assessment & Support Planning - We discuss the participant's NDIS plan, available funding, required services, preferences and support arrangements.
- Support Worker Matching - Where appropriate, we identify a suitable support worker while considering participant preferences.
- Service Commencement - Once the service arrangement has been agreed and confirmed, supports can commence subject to availability.
- Ongoing Communication - We maintain communication regarding support arrangements and respond to changes in participant circumstances or requirements.
